Nakia Grimes simply wanted to get her birth certificate corrected after noticing it wrongly listed her as a man.
But when she contacted public records officials she was told she had to provide test results of an invasive smear before the certificate would be changed.
Without the change Nakia, 37, could not renew her driving licence but the official insisted she handed in evidence of her gender.
The mother of Clayton County, Georgia, US said: "When I went to retrieve my birth certificate I let her know that the sex status is wrong.
"I'm a woman, was born a woman."
But the official dismissed her claim, leaving Grimes angry and embarrassed about having to call her doctor.
Grimes added: "She said I needed to have a PAP exam, have a doctor write a note verifying you're a woman, and bring it back -- notarised."
After Grimes took her case to local media, the director of the record office insisted the clerk had not followed protocol, issuing Grimes with a corrected birth certificate after checking that of her son.

A Georgia state worker has some sex-plaining to do after she reportedly demanded that a woman prove her sex to get a driver's license.
